Requesting Party/Research Facility/Sponsor (also referred to as “I/we”), the authorized signor(s) of the above credit card (or if paid by wire transaction) understand and consent to the Agreement to purchase Peptides, including GLP-1s and other compounded medications. Upon release of the products, Summit Medical Solutions has NO obligation to refund any monies.
